Medication Errors: How Really Smart People, (like nurses) Make Really Stupid Mistakes

I've been a nurse for over 25 years. Healthcare in the USA is not safe for patients or nurses! The cost in lives, suffering, and money due to medical error is shameful. Often times there are not enough nurses to provide care in a skilled and compassionate way. Worse, the work environments we practice in are often chronically understaffed and we can't do things the way we are supposed to. The more consumers and healthcare professionals understand about medical errors and nursing work, the better we will be able to speak up about concerns and advocate for staff, assertiveness training, effective root cause analysis, and safety engineering. This youtube helps to explain how easy it is to make a mistake. It is important because it sheds light on how the systems fails to support the work nurses do rather than the status quo of blaming the nurse.
