Are you a caretaker or a loved one of someone who is losing their memory?
Living with Alzheimer's disease

Alzheimer's disease and dementia-related conditions affect millions of lives worldwide. Alzheimer's is one of the top ten causes of death in America. It cannot be cured, slowed or treated, according to the Alzheimer's Association. The progressive disease steals away not only a person’s memory, but eventually the ability to care for themselves.

Are you caring for a loved one who has Alzheimer's disease or a dementia-related illness? What has your experience been like ? What have you learned about the disease, your loved one and about yourself? We invite you to share your personal stories with CNN.

Tune in Sunday, June 28 at 9 p.m. ET/PT when CNN Films presents "Glen Campbell ... I’ll Be Me". The documentary follows Campbell and his family struggle with the diagnosis and progression of the illness through his poignant “Goodbye Tour.”
